About this service
Formal demand for indoor case papers, OT notes, nursing charts and investigation reports under the Clinical Establishments (Registration and Regulation) Act 2010 rules that require hospitals to hand records to the patient within 72 hours. For government hospitals, we route the demand as an RTI application under the RTI Act 2005 to the PIO of the Medical Superintendent's office, with a pre-filled first-appeal template if the initial request is refused.
